NJLEND Stakeholder Needs Assessment Survey

The Boggs Center on Disability and Human Development is in the process of applying for continued federal funding from the Maternal and Child Health Bureau, Health Resources & Services Administration, to support our New Jersey Leadership Education in Neurodevelopmental and Related Disabilities (NJLEND) program. As part of this application, we’re inviting people with developmental disabilities and family members to complete a brief survey about their experiences accessing and receiving health care and other services and supports. The results will be used to ensure that our proposal reflects the needs of those in our state.
If you’d like to participate in this survey, please submit your response by Wednesday, May 13th. Please feel free to distribute this survey broadly to your networks that include families of children and young adults with autism spectrum disorder (ASD) and other developmental disabilities.
