Training & Consultation Specialist for PCAST and Pathways to Postsecondary Success

The Boggs Center on Disability and Human Development at Rutgers Robert Wood Johnson Medical School is seeking a Training and Consultation Specialist for PCAST and Pathways to Postsecondary Success.

Under the supervision of a faculty member, this position coordinates planning, development, training and technical assistance activities related to best practices in Person-Centered Approaches in Schools and Transition (PCAST) and Pathways to Postsecondary Success. Provides training and technical assistance on best practices in Person-Centered Planning (PCP), transition to adult life, integrated community employment, program design, program implementation and program enhancement to school personnel statewide, with concentration on person-centered approaches for all students with disabilities. Responsibilities include collaborating with the New Jersey Department of Education’s Office of Special Education (DOE-OSE) and other Boggs Center personnel conducting training and technical assistance in PCP; post-secondary success, developing, planning, and providing trainings; designing and disseminating instructional and other resources; project specific data analysis and reporting; and, responding to requests for information.

Licensed Practical Nurse

The Boggs Center on Disability and Human Development at Rutgers Robert Wood Johnson Medical School is seeking a Licensed Practical Nurse for the Family Medicine Resource Unit serving people with Intellectual and Developmental Disabilities.

The Licensed Practical Nurse, in collaboration with physicians, resident physicians, nurse practitioners, the designated Resource Unit supported Medical Assistant, and other members of the healthcare team, coordinates care for patients with Intellectual and Developmental Disabilities (I/DD) who are service recipients of NJ Division of Developmental Disabilities (DDD). The Licensed Practical Nurse supports the education of resident physicians through care coordination, which requires significant interaction with community disability service provider agencies, including direct support professionals and program directors, families, and specialists in a range of disciplines. In coordinating services for this vulnerable population, many of whom have communication and other functional limitations, the Licensed Practical Nurse serves as a bridge between patients, resident/attending physicians, families, and community service providers.
